We check all three and fix whatever's failed.
Where the pointing has gone but the stack is sound, repointing in a matching mortar restores it and keeps water out. Where brickwork has spalled or the stack is leaning, a partial or full rebuild is the proper fix. Look for gaps or crumbling mortar between the bricks, sandy mortar dropping into the gutter, or damp appearing on the chimney breast inside. If you can see daylight through the joints or the pointing is visibly receded, it's time. A free survey will confirm it.
